Results by Mail, Email, Fax

If you would like to recieve your results by Mail, Email, or Fax will we need to have a signed copy of a HIPPA form. Call our Results on how to submit your HIPPA form so we can send your results.

As we continue to enhanced our Service provided to you we online Resulting on our Website. For the Time being you will have to Call in to get your results. At that point we can also send you a hard copy of your results after we have received your HIPPA form.
